File a report 

The "Broken Arrow Operator’s Traffic Collision Report Form” is to be completed by parties involved in traffic collisions to document the event when a police officer does not investigate the incident. Once completed, the form can be sent to the Broken Arrow Police Records Division to be retained as documentation of the incident.

The “DPS Motor Vehicle Collision Financial Responsibility Form” is to be completed and forwarded to the Oklahoma Department of Public Safety by any driver involved in a traffic collision and settlement of the collision has not been reached within six months of the date of the collision.

Get a copy of an Online Collision Report

I need to obtain a collision report. 

The Broken Arrow Police Department has made completed automobile collision reports available online, for free, to parties that were involved in an accident.

Effective June 2018, BAPD is digitizing its crash reports through a partnership with the Carfax Police Crash Assistance Program. The reports are stored electronically in a secure database and available online at www.crashdocs.org.

Reports needed by involved parties, insurance companies and other members of the public are now easily obtained online within minutes from home or office.
